ProMedical Personnel are currently seeking a CMHN.
The Community Psychiatric Nurse (CPN) will participate in the provision of a community mental health service to HC personnel. This will involve skilled assessment of mental health state, risk assessment and occupational needs assessment to direct appropriate management actions.
3. The CPN will work within the Nursing and Midwifery Council (NMC) Code of Professional Conduct, and all NMC and MOD Standards, Policies and Guidelines.
PRIMARY DUTIES
4. To function as an autonomous practitioner with the following roles:
a. Initial assessment of mental health needs of referred individuals.
b. To manage the HC caseload of patients in conjunction with the other HC CPN, implementing and evaluating care plans.
c. To maintain accurate records and clinical notes following all client contact in accordance with NMC Guidelines.
d. To provide appropriate reports to referring Medical Staff, and, with the clients consent to other professional agencies (including the chain of command (CoC)) as may be required. This should happen on initial assessment, discharge and when clinically or occupationally necessary.
e. Liaise with and advise the CoC and other authorities regarding the specific management of individuals or groups, and to provide advice to primary care regarding occupational grading.
f. Participate in weekly multi-disciplinary team supervision with DCMH Plymouth.
g. Provide input to the weekly carers meeting at HC.
h. Participate in other clinical and management meetings as required by DCMH Plymouth, RNSQ HMS DRAKE and HC.
i. To provide mental health promotion activities for HC personnel.
j. To provide professional support to HC staff as requested by OC HC (however clinical treatment of HC staff will be the responsibility of other DCMH Plymouth Clinicians).
k. To recognise the limits of personal competence and seek clinical supervision and advice as appropriate.
SECONDARY DUTIES
5. The following constitute the additional roles expected of the post holder:
a. Maintain data regarding clinical and educational work, by forwarding clinical contact sheets to DCMH Plymouth for inclusion as database records.
b. To undertake and participate in appropriate research projects, whilst taking into account ethical, practical and managerial considerations.
c. Participate in continuous development, operation and evaluation of the Mental Health Provision to HC.
d. Participate in the assessment of service need and planning of future services.
